Case Management Q and A
Q: Who is eligible for Case management?
A: An individual who is at least 60 years old
and needs two or more services to assist them in his or her home.
Q: How does it work?
A: A Case Manager will visit you in your home
to complete a comprehensive assessment. An individualized service
plan will be developed with you to address your needs. The Case
Manager will assist in arranging for services and continue to be
available to you as other needs arise.
Q: Who will make the decisions?
A: You and your family will decide which
agencies or organizations will be involved, and which services you will
accept.
Q: What does Case Management cost?
A: There is no cost to you to
participate in the Case Management Program. The agencies providing
the services you choose may charge for their help. Your Case Manager
will assist in finding out if you are eligible for financial assistance
to pay for your services. Some individuals receiving Case
Management are eligible for the Elderly Waiver Program to pay for
services and medical expenses.
Q: What is the Elderly Waiver?
A: It is a Medicaid program available through
the Iowa Department of Human Services to people aged 65 and older who
meet medical, income and resource guidelines. Your Case Manager
can assist you to explore the Elderly Waiver as a payment source.
Q: What can't Case Management do?
A: Case Management can't provide
24-hour-a-day care or keep everybody out of a nursing home. The
Case Manager does not provide direct care to you; her job is to
coordinate the services that come to your home, and to periodically
ensure your well-being.
